Camp/School Forms

If your child is having an annual examination in May, June, July or August, please bring the forms with you. Your doctor will often be able to fill them out at the time of the visit.  Otherwise, please send us the forms as soon as you have received them together with a self- addressed/ self stamped envelope. All forms MUST have the childs full name and all demographic information filled out. The forms will be completed as soon as possible (PLEASE allow for a minimum of one-week turnaround time for completion of the forms.) and they will be sent back to you by the means designated (either directly to the school/camp or back to you).

Adoption Services

Dr. Melissa Goldstein provides general pediatric care for children adopted internationally and domestically. Her adoption practice focuses on health issues specific to previously orphaned children. Dr. Goldstein helps families through the process of adoption, including performing pre-adoption medical evaluations and providing guidance to families through the challenging process of adopting a child both from a foreign land (travel medications and vaccines, new parent coaching, and cultural obstacles) and within the United States.

International Medical Review

Sibling groups at reduced rate can be discussed with office.

 

Includes review of video photos for fetal alcohol syndrome and all updates and consultation from abroad. Does not include in-person medical examination.

Domestic Adoption

 $1,000 includes 2 birth mother          match reviews.

Each additional referral at a               reduced rate.

 Includes analysis of pre-natal records, review  of medical/social history of birth family,    consults with obstetrician, pediatrician.  Does  not include in-person medical examination.

Expedited Review

Within 24 hours, an extra $300
Within 48 hours, an extra $150

 

Pre-Adoption Talk: Prior to receiving any referral: A session to help families begin and understand the adoption process (domestic or international). If applicable, includes check list of acceptable special needs.

Initial adoption visit (Dr. Goldstein examines the child upon arrival in the US).

Includes: developmental screening, physical exam, parenting education, blood drawing and interpretation of results, and management of medical findings.
