We are pleased to inform you that CHP will be providing Pfizer Vaccine booster for children ages 12 years and up who are EXISTING registered patients in our practice. At this time, non-CHP patients must go to another vaccine site.
A specific appointment must be scheduled for the administration of the vaccine. The only exception is if you have a routine physical exam scheduled, we will offer it at that visit as well.
We have limited supply but expect to continue to receive doses weekly as we use up our allotment.
We are glad to be able to provide this service to you, however it is a big undertaking to do this safely and efficiently in our office. Please bear with us, as this is a time-consuming process for our staff, and we have limited space.
If you have specific COVID vaccination questions regarding your child(ren) we are available for tele-medicine consultation at least one day prior as the vaccine appointment is ONLY to administer the vaccine.
Prior to scheduling a Covid vaccine appointment, you must email and complete the following:

- COVID Immunization Screening & Consent Form
- NY State COVID-19 Vaccine Form
- Both sides of your current insurance card
Please come on time to your appointment. If you need to cancel, we ask that you please do so 24 hours ahead of time so we can give your time slot to another family.
